É comum que haja alopécia parcial ou total na área do crânio após contato com a radiação, podendo ser do tipo permanente se a dosagem de radiação for alta. A alopécia é causada diretamente pelo feixe de radiação que atinge o couro cabeludo e, portanto, pode se tornar irregular por isso mesmo. Cabelos possuem uma baixíssima resistência à tração e tendem a cair rapidamente tanto em homens, quanto em mulheres.
O grau de gravidade e de permanência deste tipo de alopécia está diretamente relacionado à dosagem de radiação. Por sorte, em muitos casos, trata-se de uma forma reversível de calvície, que pode ser solucionada de modo autônomo e espontâneo nos meses seguintes após encerrar-se o tratamento.
